深圳市振东招标代理有限公司受招标人委托对下列产品及服务进行国际公开竞争性招标，于 2016-04-19 在中国国际招标网公告。本次招标采用传统招标方式，现邀请合格投标人参加投标。

3、投标人资格要求

投标人应具备的资格或业绩：

1) 投标人必须是所投产品的制造商或合法代理商或制造商针对本项目直接授权的供应商（提供授权书复印件并加盖公章）（见格式Ⅳ— 9 － 4 ）；

2) 投标人必须是独立法人机构，具有相关经营范围（且通过年检提供复印件加盖投标人公章）；

3) 投标人具有的针对所投产品的《医疗器械生产企业许可证》或《医疗器械经营企业许可证》（提供复印件并加盖公章）；

4) 投标人所投产品必须具有国家食品药品监督管理局颁发的《医疗器械产品注册证》，如是进口设备，需进口医疗器械注册证，（提供以下证明资料之一：①提供有效的《医疗器械产品注册证》，或者《进口医疗器械产品注册证》；②提供《医疗器械产品注册证》和注有所投产品生产日期证明的原件扫描件，原件备查）。注册证须含附件：医疗器械产品生产制造认可表或医疗器械产品注册登记表）；

5) 证明投标人已具备履行合同所需的财务、技术和生产能力的文件；

6) 投标人基本账户开户银行在开标日前三个月内开具的资信证明原件或该原件的复印件；

7) 投标人所投产品的销售业绩，须提供设备在国内的销售业绩表及合同复印件或中标通知书复印件（表中应列出设备的出厂日期、用户名称、地址、联系人及联系方式等资料）；

8) 具有深圳市政府采购注册供应商资格（供应商注册网址： http://www.szzfcg.cn ）。
